It shows how to integrate universal screening, structured suicide risk assessment and immediate safety planning—plus lethal means safety counseling—into routine adolescent visits. It clarifies warm handoffs and referrals to behavioral health and crisis services, and defines a proactive follow-up plan with timely outreach, scheduled touchpoints and ongoing monitoring. The goal is a consistent, evidence-informed workflow that improves how teams identify and respond to suicide risk.
