They\u2019re positioned as the reason the pregnant lady cries, the teenager recoils in angst, and the premenstrual woman eats ice cream straight from the gallon. Effects on mood aside, however, hormones are essential for life. Among many things, they are also the reason a pregnancy can be sustained, why kids grow, and how our bodies balance the electrolytes necessary for survival. Halker Singh, MD<\/a>, neurologist and associate professor of neurology at Mayo Clinic with a special interest in understanding the gender differences in migraine <\/li>\n<\/ul><\/div>\n<hr\/><\/div>\n<p>In childhood, people of both sexes are at equal risk for migraine attacks, but after puberty, women are <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27836433\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27836433\/\"><span>two to three times more likely<\/span><sup>1<\/sup><\/a> to experience them. And according to the American Migraine Foundation, <a href=\"https:\/\/americanmigrainefoundation.org\/resource-library\/what-is-migraine\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/americanmigrainefoundation.org\/resource-library\/what-is-migraine\/\">women are three times more likely than men<\/a> to live with migraines, with their first attack often occurring around the time they get their first period.<\/p>\n<p>A female person&#8217;s hormonal state will not only fluctuate in monthly cycles, but also throughout their lifetime, with changes like pregnancy, postpartum, perimenopause, and menopause. (Technically, migraine is the condition, and a migraine attack is the result of the condition.) \u201cCertain genes give your brain a vulnerability to have migraine attacks,\u201d she says. \u201cBut whether you actually get the attacks also depends on a number of different <a href=\"https:\/\/www.cdc.gov\/genomics\/disease\/epigenetics.htm\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.cdc.gov\/genomics\/disease\/epigenetics.htm\">epigenetic factors<\/a>, or factors within and outside your body [including <a href=\"https:\/\/www.wellandgood.com\/list-of-migraine-triggers\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/list-of-migraine-triggers\/\">lifestyle<\/a> and <a href=\"https:\/\/www.wellandgood.com\/migraine-triggers-weather\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/migraine-triggers-weather\/\">environmental variables<\/a>], and one of those really powerful factors is [the female sex hormone] estrogen.\u201d<\/p>\n<p>Dr. Munro clarifies that while the common <a href=\"https:\/\/www.wellandgood.com\/migraine-denial\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/migraine-denial\/\">perception of migraine is that it\u2019s just a \u201cbad headache,<\/a>\u201d it can encompass a lot more than that. Things like <a href=\"https:\/\/www.wellandgood.com\/migraines-and-sleep\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/migraines-and-sleep\/\">too much or too little sleep<\/a>, or a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9141901\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9141901\/\"><span>spike in blood glucose levels<\/span><sup>3<\/sup><\/a> can contribute to the development of a migraine attack. It isn\u2019t often just one thing that will trigger a migraine, she adds; it\u2019s a combination of triggers stacking on top of one another that begins the roll of an attack, and among them are various, often inescapable changes in female reproductive hormones.<\/p>\n<h2>How hormones can cause or contribute to migraines<\/h2>\n<p>One theory posits that the fluctuating hormones of the menstrual cycle might just irritate or set off the naturally sensitive migraine brain, <a href=\"https:\/\/www.hopkinsmedicine.org\/health\/conditions-and-diseases\/headache\/how-a-migraine-happens\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.hopkinsmedicine.org\/health\/conditions-and-diseases\/headache\/how-a-migraine-happens\">putting women at higher risk of a migraine attack<\/a>.<\/p>\n<p>It\u2019s also possible that in some people with migraine, quicker or more precipitous <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4932235\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4932235\/\"><span>drops in estrogen<\/span><sup>4<\/sup><\/a> at the end of their menstrual cycle may make the brain more sensitive to <a href=\"https:\/\/www.everydayhealth.com\/prostaglandins\/guide\/\" target=\"_blank\" rel=\"noopener nofollow\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.everydayhealth.com\/prostaglandins\/guide\/\">prostaglandins<\/a>, or hormone-like chemicals tied to pain and inflammation, says Dr. Munro.<\/p>\n<p>Some research has shown that transgender women on estrogen therapy are <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9325838\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9325838\/\"><span>more likely to have migraines than cisgender men<\/span><sup>5<\/sup><\/a>, and conversely, that transgender men on testosterone therapy have fewer migraines than cisgender women\u2014all of which highlights the role of changing levels of estrogen when it comes to the risk of a migraine attack.<\/p>\n<h3>During menstruation<\/h3>\n<p>Estrogen and progesterone fluctuate throughout the menstrual cycle, with both hormones typically plummeting at the very end of the cycle, prior to the start of menstrual bleeding (and of the next cycle). \u201cWhat the studies seem to indicate is that it&#8217;s <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10512516\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10512516\/\"><span>falling levels of estrogen at the end of the menstrual cycle<\/span><sup>6<\/sup><\/a> that are particularly powerful in aggravating the vascular pathway that triggers migraine attacks,\u201d says Dr. Munro.<\/p>\n<blockquote>\n<p>\u201cFalling levels of estrogen at the end of the menstrual cycle are particularly powerful in aggravating the vascular pathway that triggers migraine attacks.\u201d \u2014Dr. Munro<\/p>\n<\/blockquote>\n<p>Indeed, of the women who experience migraines, <a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/8260-menstrual-migraines-hormone-headaches\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/8260-menstrual-migraines-hormone-headaches\">about 60 to 70 percent<\/a> notice a\u00a0<a href=\"https:\/\/www.wellandgood.com\/migraine-and-period\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/migraine-and-period\/\">connection between their migraines and menstruation<\/a>, seeing an uptick in their migraines on the days leading up to their period and\/or during the first three days of their period, when estrogen and progesterone are at their lowest.<\/p>\n<p>Research shows that about <a href=\"https:\/\/thejournalofheadacheandpain.biomedcentral.com\/articles\/10.1007\/s10194-010-0197-0\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/thejournalofheadacheandpain.biomedcentral.com\/articles\/10.1007\/s10194-010-0197-0\"><span>2.7 percent of people have pure menstrual migraines<\/span><sup>7<\/sup><\/a> (PMM), meaning they only have migraines around the time of their period, whereas 4.6 percent have menstrually related migraines (MRM), meaning they get migraines <a href=\"https:\/\/mcpress.mayoclinic.org\/healthy-brain\/estrogen-fluctuations-and-migraines-the-menstrual-connection\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/mcpress.mayoclinic.org\/healthy-brain\/estrogen-fluctuations-and-migraines-the-menstrual-connection\/\">more frequently around their period<\/a>\u00a0but also have them at other times in the month. \u201cWe don\u2019t fully understand why some women get pure menstrually related attacks and others have attacks both around menstruation and at other times in the month,\u201d says Dr. Munro. \u201cIt is part of the complexity of migraine that makes it a challenge for the individual to search for their own solution.&#8221;<\/p>\n<h3>Hormonal contraception<\/h3>\n<p>Because the migraine brain is, again, sensitive to change, <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/chronic-daily-headaches\/in-depth\/headaches\/art-20046729\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/chronic-daily-headaches\/in-depth\/headaches\/art-20046729\">fluctuating estrogen levels<\/a> can be a trigger for more migraine attacks, whereas stable estrogen levels may reduce the number of attacks. As a result, hormonal birth control may either increase or decrease the frequency of migraine attacks, largely depending on whether it\u2019s causing more or less hormonal change than the person\u2019s baseline, says family physician <a href=\"https:\/\/www.onemedical.com\/providers\/navya-mysore\/\" target=\"_blank\" rel=\"noopener nofollow\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.onemedical.com\/providers\/navya-mysore\/\">Navya Mysore, MD<\/a>, who is a chronic migraineur herself and the national program medical director of sexual and reproductive health at One Medical. \u201cIt really depends on the patient and their response to the birth control.\u201d<\/p>\n<p>Dr. Mysore adds one important note: Combined hormonal birth control\u2014which contains both estrogen and progesterone\u2014is contraindicated for people who get migraine with <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/migraine-with-aura\/symptoms-causes\/syc-20352072\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/migraine-with-aura\/symptoms-causes\/syc-20352072\">aura<\/a> (or temporary visual or other sensory disturbances that occur before a migraine attack strikes) as <a href=\"https:\/\/migrainetrust.org\/live-with-migraine\/self-management\/migraine-and-hormones\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/migrainetrust.org\/live-with-migraine\/self-management\/migraine-and-hormones\/\">it can increase the risk of stroke<\/a>. (Note that all hormonal IUDs are progesterone-only, meaning <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2938905\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2938905\/\"><span>they are considered safe for people who experience auras<\/span><sup>8<\/sup><\/a>.)<\/p>\n<p>For people who get migraines<em> without <\/em>aura, however, \u201cit may be worth trying a hormonal birth control method [containing estrogen] to see if it might help, especially if you notice that your episodes are around ovulation or right before getting your period,\u201d says Dr. Mysore.<\/p>\n<h3>Pregnancy<\/h3>\n<p>\u201cPregnancy is a time where hormones are typically stable and not fluctuating,\u201d says neurologist <a href=\"https:\/\/www.mayo.edu\/research\/faculty\/halker-singh-rashmi-b-m-d\/bio-20147050\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.mayo.edu\/research\/faculty\/halker-singh-rashmi-b-m-d\/bio-20147050\">Rashmi Halker Singh, MD<\/a>. It is common for migraine attacks to continue during the first trimester of pregnancy, \u201cbecause hormone levels are continuing to change and build,\u201d she says. But in the second and third trimesters, the majority of people with migraine will <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2440903\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2440903\/\"><span>notice a decrease in their migraine attacks<\/span><sup>9<\/sup><\/a>.<\/p>\n<p>That said, there is one subset of people with migraine who may not experience this pregnancy-related dip: those who get auras, who are actually more likely to be triggered when in a high estrogen state like that of pregnancy, says Dr. Halker Singh. (For the same reason, some people who have never had an aura with their migraine attacks can also <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8311834\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8311834\/\"><em>start <\/em><\/a>.)<\/p>\n<h3>Postpartum<\/h3>\n<p>In a study that followed women with a history of migraines during and after pregnancy, <a href=\"https:\/\/journals.sagepub.com\/doi\/10.1046\/j.1468-2982.2003.00480.x?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed\" target=\"_blank\" rel=\"noopener nofollow\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/journals.sagepub.com\/doi\/10.1046\/j.1468-2982.2003.00480.x?url_ver=Z39.88-2003&amp;rfr_id=ori:rid:crossref.org&amp;rfr_dat=cr_pub%20%200pubmed\">55 percent noticed a return of their migraines<\/a> within the first month postpartum. The huge drop in estrogen and progesterone post-birth is likely responsible for that resurgence of migraine attacks, says Dr. Mysore. As a result of this major hormonal swing, women who have never experienced a migraine before may also start experiencing them in the postpartum period.<\/p>\n<p>It\u2019s not just the hormones that increase the risk for migraine attacks during the postpartum period, though: That cute baby is also responsible for the sleep deprivation, the stress, and the fact that dinner might be a Reese\u2019s Peanut Butter Cup washed down by the cold coffee you never got a chance to drink during the day. Such behavioral and lifestyle changes\u2014like missing out on sleep and skipping meals\u2014fall into the bucket of common migraine triggers themselves.<\/p>\n<p>While it\u2019s true that<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10147488\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10147488\/\"><span> breastfeeding may have a protective effect against migraine attacks<\/span><sup>12<\/sup><\/a> (due to the ways in which lactation keeps estrogen levels steady and delays the return of menstruation), the exhaustion and stress that can come along with it may <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3974500\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3974500\/\"><span>negate those benefits<\/span><sup>13<\/sup><\/a>.<\/p>\n<h3>Perimenopause<\/h3>\n<p><a href=\"https:\/\/www.wellandgood.com\/perimenopause-definition\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/perimenopause-definition\/\">Perimenopause<\/a> is the time during which the body transitions to menopause, and levels of reproductive hormones (like estrogen) begin to drop. It generally <a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/21608-perimenopause\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/21608-perimenopause\">begins when a person is in their forties<\/a> but can start earlier or later. The associated decrease in estrogen levels\u2014which often occurs erratically throughout the transition\u2014can certainly result in an uptick in migraine attacks, says Dr. Halker Singh.<\/p>\n<p>But Dr. Munro points out that just as in postpartum, it\u2019s not solely the hormones that can cause migraines to surge during this phase of life; it can also be the related lifestyle and behavioral changes. In the case of perimenopause, you might be <a href=\"https:\/\/www.wellandgood.com\/sleep-problems-perimenopause\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/sleep-problems-perimenopause\/\">dealing with difficulty sleeping<\/a> or the stress of big life changes (perhaps a job promotion), the loss of an elderly parent, or parenting a teenager.<\/p>\n<p>But don\u2019t lose all hope: In many cases, migraine attacks <em>do<\/em> decrease in menopause (more on that below). Then again, the <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6082400\/\" target=\"_blank\" rel=\"noopener\" referrerpolicy=\"no-referrer-when-downgrade\" class=\"no-underline citation\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6082400\/\"><span>median length of perimenopause is four years<\/span><sup>14<\/sup><\/a>; and for some unlucky souls, it can last longer than 10 years.<\/p>\n<h3>Menopause<\/h3>\n<p><a href=\"https:\/\/www.wellandgood.com\/facts-about-menopause\/\" referrerpolicy=\"no-referrer-when-downgrade\" data-vars-event=\"body text\" data-vars-click-url=\"https:\/\/www.wellandgood.com\/facts-about-menopause\/\">Menopause<\/a> is technically defined as the point in time 12 months after a person\u2019s last menstrual period, when their reproductive capacity has ended. After the erratic fluctuations of perimenopause, hormones tend to level out in menopause, and for many, migraine attacks decrease as a result. Even for those in whom migraines don\u2019t disappear completely, Dr. Munro notes that \u201cthey may be easier to treat or less troublesome [during menopause.]\u201d<\/p>\n<p>While you may seek hormone replacement therapy (HRT) with the approval of a doctor to treat menopause symptoms that can contribute to migraines\u2014like hot flashes, night sweats, insomnia, and many others\u2014the use of HRT can also cause <em>more<\/em> migraine attacks by triggering hormone fluctuations, says Dr. Munro, so it\u2019s important to work with a specialist to find the right medication for you.<\/p>\n<h2>Are other hormones responsible for triggering migraines?<\/h2>\n<p>Estrogen holds a lot of power when it comes to triggering migraine attacks, but it is not the only hormone that can wreak havoc.
