According to randomized controlled trials, what is the optimal toothbrushing and \ufb02ossing sequence?<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">\u201cOver the years, it has been generally <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19138178\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">accepted<\/span><\/a><span data-contrast=\"auto\"> that the use of dental \ufb02oss has a positive effect on removing plaque. The American Dental Association (ADA) even reports that up to 80% of plaque may be removed by this method.\u201d How do we know? You can <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/11940136\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">use<\/span><\/a><span data-contrast=\"auto\"> what\u2019s called a <i>split-mouth<\/i><\/span><span data-contrast=\"auto\"> design, where each person can act as their own control\u2014for example, by flossing only one-quarter of their mouth. Researchers asked study participants to stop brushing their lower jaw so that plaque would build up, then they were randomized to floss half and leave the other half as the non-flossed control. As you can see below and at 0:37 in my video <\/span><a href=\"https:\/\/nutritionfacts.org\/video\/should-you-floss-before-or-after-you-brush\/\" target=\"_blank\" rel=\"noopener\"><b><span data-contrast=\"none\">Should You Floss Before or After You Brush?<\/span><\/b><\/a><span data-contrast=\"auto\">, not only did flossing <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/11940136\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">cut<\/span><\/a><span data-contrast=\"auto\"> plaque by about 60 percent after just three weeks, more importantly, it also halved gingivitis signs\u2014bleeding on probing and another index of gum inflammation. Note, though, that this compared flossing to doing nothing. The study subjects weren\u2019t allowed to brush their lower jaw. You don\u2019t really know until you put it to the test. What\u2019s the efficacy of dental floss in addition to a toothbrush? Surprisingly, only 3 out of the 11 studies the researchers looked at found a significant added benefit.<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The anti-flossers <\/span><a href=\"https:\/\/www.magonlinelibrary.com\/doi\/abs\/10.12968\/denu.2016.43.8.793b\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">were<\/span><\/a><span data-contrast=\"auto\"> positively giddy, comparing dentists who continued to advocate flossing in the face of the data to flat-earthers. Dentistry is a profession \u201cin denial,\u201d they wrote. \u201cOver 80% do not floss regularly and it is hard to accept that these \u2018the great unflossed\u2019 were right and we, \u2018the highly educated intelligencia\u2019 may actually have been wrong\u2026Flossing doesn\u2019t work\u2014get over it!\u201d<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">So, you\u2019ve heard of the tooth fairy. Is flossing just some tooth fairy <\/span><i><span data-contrast=\"auto\">tale<\/span><\/i><span data-contrast=\"auto\">? The review showing that only 3 out of 11 studies found a significant benefit to flossing in addition to brushing was <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19138178\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">published<\/span><\/a><span data-contrast=\"auto\"> back in 2008. Since then, more studies have <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22161438\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">found<\/span><\/a><span data-contrast=\"auto\"> that while the evidence on additional plaque reduction is weak, there is at least some evidence that flossing with toothbrushing helps with gingivitis, which is a primary reason you want to reduce plaque anyway. Why might they not have found stronger evidence? \u201cTrials were of poor quality and conclusions must be viewed as unreliable,\u201d so basically, we don\u2019t have good evidence either way because good studies really haven\u2019t been done.\u00a0<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Why not? Why wouldn\u2019t Big Floss fund the studies? It appears that all floss <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/18251638\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">works<\/span><\/a><span data-contrast=\"auto\"> the same. If you compare unwaxed to woven to shred-resistant floss, they all have about the same plaque-removal ef\ufb01cacy, something all such studies appear to have found. So, why would a floss company fund a study to show that flossing <em>in general<\/em> is good if it can\u2019t show that its product is better than another? You might just buy its competitor\u2019s floss.<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Where do we stand today? \u201cAlthough technically the evidence for \ufb02ossing <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28865811\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">is<\/span><\/a><span data-contrast=\"auto\"> weak, more importantly, the methodology and rigor of the studies examining \ufb02ossing effectiveness are also weak.\u201d For example, the studies didn\u2019t <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28501067\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">assess<\/span><\/a><span data-contrast=\"auto\"> the frequency or quality of people\u2019s flossing. (You can <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/30611058\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">see<\/span><\/a><span data-contrast=\"auto\"> an incorrect way of flossing below and at 3:18 in my <\/span><a href=\"https:\/\/nutritionfacts.org\/video\/should-you-floss-before-or-after-you-brush\/\" target=\"_blank\" rel=\"noopener\"><b><span data-contrast=\"none\">video<\/span><\/b><\/a><span data-contrast=\"auto\">.) The bottom line is that the American Dental Association continues to <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28865811\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">recommend<\/span><\/a><span data-contrast=\"auto\"> brushing and flossing every day. But what\u2019s the proper sequence? Should you floss before or after you brush?\u00a0<\/span><\/p>\n<p><img decoding=\"async\" loading=\"lazy\" class=\"aligncenter size-full wp-image-95730\" src=\"https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18.png\" alt=\"\" width=\"1920\" height=\"1080\" srcset=\"https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18.png 1920w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-960x540.png 960w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-1024x576.png 1024w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-768x432.png 768w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-1536x864.png 1536w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-480x270.png 480w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-1200x675.png 1200w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-720x405.png 720w, https:\/\/nutritionfacts.org\/app\/uploads\/2023\/09\/3-18-540x304.png 540w\" sizes=\"auto, (max-width: 1920px) 100vw, 1920px\"\/><\/p>\n<p><span data-contrast=\"auto\">\u201cSome dentists <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29741239\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">argue<\/span><\/a><span data-contrast=\"auto\"> that \ufb02ossing should come \ufb01rst because you stir up the particles and plaque that the toothbrush can brush away subsequently. The \ufb02uoride from your toothpaste is also more likely to reach the interdental areas [the spaces between teeth] if food wedged in between the teeth is removed before brushing.\u201d Others, however, \u201crecommend brushing be done \ufb01rst and their rationale is that brushing teeth first removes the bulk of the particles on the teeth and flossing afterward can remove interdental plaque better. It could also force the remaining bit of fluoride that is left on the teeth from toothpaste into the interdental spaces.\u201d You don\u2019t know until you put it to the test.<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">An article titled \u201cThe E<\/span><span data-contrast=\"auto\">\ufb00<\/span><span data-contrast=\"auto\">ect of Toothbrushing and Flossing Sequence on Interdental Plaque Reduction and Fluoride Retention: A Randomized Controlled Clinical Trial\u201d <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29741239\/\" target=\"_blank\" rel=\"noopener\"><span data-contrast=\"none\">reported<\/span><\/a><span data-contrast=\"auto\"> that flossing first won\u2014both in terms of getting rid of significantly more plaque, as well as getting more of the fluoride between the teeth. \u201cWhen we use dental \ufb02oss after brushing, much of the particles that are removed by dental \ufb02oss would stay in place.\u201d The bottom line is that \ufb02ossing followed by brushing is preferred.<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">What about oil pulling? Short answer: Don\u2019t do it.
